I doubt it's really stunning his doctors, it's not unheard of, though it's certainly rare.

He'd just need a some ovary cells and some endometrial cells- it would be important that he gets checked out properly, if he sheds any of that menstrual blood into the interior of his belly he could die.

I've seen some unusual intersex configurations before, though most did not menstruate- in fact, that's how some of it was diagnosed...
