The debunking can be found in an issue of JAMA. I can't remember the exact issue, because it's been a while. I want to say 1Q90, but my brain's sense of place (where I was when I read the article, and the circumstances in which I remember reading it) instead points to 1Q89. It's been confusing me for years, and I haven't found anyone with an online subscription who can look it up for me...
